Journey to Wellness in the Transformational Program
Interfaith Dental’s Transformational Program provides ongoing, comprehensive dental care with a focus on
restoration and oral health education and prevention.
Applications for Transformational Care
The Transformational Program is for eligible patients who qualify for the services. To find out if you qualify for care, you must submit an application. Because Interfaith Dental is a nonprofit organization, we can only accept a limited number of new patients into the Program at a time.
Before You Apply
Please have the following documents ready before starting your application:
- Proof of income (such as your two most recent pay stubs, tax return, other assistance)
- Proof of address (such as a utility bill or vehicle registration)
- Valid photo ID
- Any medical and/or dental insurance cards
Applications that are missing required documents cannot be processed.
How to Apply
- Complete the online application.
- Upload all required documents. Applications cannot be reviewed until all required information has been received.
- Click SUBMIT on the application for review.
Ready to Apply?
To help give everyone a fair opportunity to apply, applications are opened on Monday for both our Greater Nashville and Rutherford County locations. Once all available spots are filled, applications will close until additional openings become available.
Application days and times may change based on community need and program capacity. We encourage you to check back daily. For quickest response, please apply using the links below.
If all spots are filled but you are over 65, under the age of 18, or pregnant, please call 615-667-6651 to speak with our New Patient Intake Coordinator.
If language is a barrier or you are unable to complete the application, please call us. We have interpreter services available as needed.
