How can my organization partner with SMILE ON 65+?

Our Community Dental Health Coordinators are available to provide training to community partner staff on the importance of oral health care for older adults as well as what options are available for older adults that need dental care, including the SMILE ON 65+ program. 

Community Dental Health Coordinators can also provide oral health care education directly to groups of older adults. If you are interested in having an educational presentation or a Dental Bingo at your site, please contact Amy Ayers, Program Administrator at amy@smileon65plus.com.

Organizations may also refer clients that appear eligible to the program using our Referral Form or having them call our hotline at 1-800-509-6055. Clients will be pre-screened for eligibility and connected with their closest available partner clinic to verify eligibility. 

 

Is my client eligible for SMILE ON 65+?

SMILE ON 65+ client eligibility criteria:

  • The client must be a minimum of 65 years old;
  • be a Tennessee resident;
  • have an income level below 200% poverty, and
  • be dentally uninsured.
 

What services are offered through SMILE ON 65+? *

*Limits may apply.

  • Emergency Care, X-Rays, and Exams
  • Fillings
  • Dentures and Partials
  • Crowns
  • Root Canals
  • Cleanings and Preventive Care
  • Periodontal Treatment (gum disease)
  • Transportation assistance in the form of travel vouchers are available based on need
  • Case Management, through Community Dental Health Coordinators (CDHCs), is available to help older adults navigate available services and eliminate barriers to treatment

*Services not covered by SMILE ON 65+ may be offered by the clinic on a sliding fee scale.

 

What is the cost to receive services in SMILE ON 65+?

Older adults that meet eligibility will be asked to contribute a facility fee of $25 at each appointment for covered services.

This program is funded under a grant contract with the state of Tennessee.
